Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. Reports to: Waiver Service Coordinator Supervisor

Job Duties:

  • Team Lead, WSC Buckeye Healthcare works in conjunction with the Program Supervisor on training and team development.
  • Provide education and support to Buckeye Waiver Service Coordinators, members and their caregivers.
  • Preceptor to new staff; primary train the trainer with computerized program platforms etc.
  • Conducts phone call and in-person contacts with clients in accordance with program specifications to monitor the delivery of all services in the client's service plan as needed.
  • Respond to urgent/emergent needs for a Waiver Service visit if the assigned Waiver Service Coordinator is not available.
  • Authorizes changes in amount, scope, and duration of waiver services.
  • Collaborate with the Departments of Job and Family Service to establish and/or maintain Medicaid eligibility.
  • Assist Case Aides with issues related to Medicaid, transportation and service planning.
  • Work with internal and external entities to resolve issues related to member program assignment. Assist in transitioning dual eligible members into the Integrated Care Delivery System aka MyCare Ohio as appropriate
  • Evaluate assessment and Waiver Service Coordinator data to determine ongoing program eligibility, care needs, and service options.
  • Advocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings in collaboration with providers of service(s).
  • Supports the AAA mission, vision and strategic plan, goals and management decisions.
  • Utilize clinical judgment and perform clinical practice based upon education.
  • Abide by the professional code of ethics established by licensure. As required by Ohio Law report abuse, neglect, or exploitation of older adults as a mandated reporter.
  • Assist with and completes special projects and activities as assigned and communicate with management issues/concerns/suggestions to improve processes.
  • Meet time frames for timely and accurate completion of documentation and maintain member's confidentiality according to agency policy.
  • This position's primary duty is to perform work directly related to the successful operation of the Agency. Accordingly, the employee must exercise discretion and independent judgment with respect to matters of significance.
  • All staff have the responsibility and authority to propose ways to improve quality and to fully participate in quality improvement efforts.
  • Perform other duties as assigned by supervisor as needed to meet agency goals.
  • Work in conjunction with the Program Supervisor to streamline processes and develop workflows.
  • Monitor Program Supervisor's and Buckeye voicemail messages in their absence.

Qualifications:

  • Current licensure as a Social Worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing.
  • At least five years of experience working with individuals with physical or behavioral disabilities in a home health care, medical social work, or behavioral health environment is required. Some supervisory experience preferred.
  • Excellent verbal/written interviewing and assessment skills.
  • Basic knowledge of major chronic disabling illnesses, disease processes, and normal aging.
  • Ability to effectively communicate with the disabled and elderly populations and general public, both in person and by phone.
  • Ability to communicate in a collaborative, effective manner with others and to maintain good working relationships.
  • Ability to comprehend written material related to essential job functions and basic medical terminology.
  • Knowledge of local resources for older adults and persons with disabilities.
  • Ability to comprehend, recall, and apply facts related to essential job functions. Analyze, evaluate, and implement a reasonable course of action based on available information.
  • Ability to detect specific odors related to the assessment of a client's health/hygiene and the safety of the client's environment.
  • Ability to travel in a multi-county service area and access private homes and facilities which may not be readily accessible. Must have a car available during working hours, as well as the ability to legally drive a car.
  • Must have basic computer skills, including Microsoft Office applications while maintaining minimum proficiency in all requisite computer applications and software. Ability to use and transport a laptop computer with case, therefore must be able to lift/carry a minimum of 20 pounds.

Physical Requirements:

  • May sit, stand, stoop, bend, and ambulate intermittently during the day.
  • May need to sit or stand for extended periods.
  • See in the normal visual range with or without correction.
  • Hear in the normal audio range with or without correction.
  • Finger dexterity to operate office equipment required.
  • Ability to communicate in face to face, phone, email, and other communications.
  • Ability to push/pull/lift/100 pounds.
  • Requires some exposure to warm chemicals and cleaning products.
  • Requires exposures to warm and cold temperatures.
